Classifies renal cystic lesions based on CT and MRI features. Septation, wall characteristics and enhancement patterns determine Bosniak I-IV category.

Management: Benign — no follow-up needed
Thin septa, thin calcification, hyperdense cyst (<3cm)
Management: Benign — no follow-up needed
Minimally suspicious features — surveillance needed
Management: Follow-up: 6 months, then annually (5 years)
Thickened irregular wall/septa or measurable enhancement
Management: Surgery or active surveillance
Cystic mass with enhancing solid component
Management: Surgery (partial/radical nephrectomy)
